Is an indirect caregiver. Complies with other duties as described. Must be able to work collaboratively with the Multidisciplinary team. Essential Responsibilities: Evaluates and identifies members needs.Interfaces with Primary Care Physicians, Specialists and various disciplines on the development of case management plans/programs.Monitors and evaluates the effectiveness of the case management plans and modifies as necessary.Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, transfer coordination, discharge planning and obtaining all authorizations/approvals/transfers as needed for outside services for patients/families.Acts as a clinical liaison, per their specialty, with outside agencies such as County CCS, non-plan facilities, outside providers, employers and/or workers compensation carriers and third party administrators.Prepares reports, communicates program changes to appropriate staff and develops protocols in accordance with state regulations.Acts as a patient advocate and educator to assure that the patient has the knowledge to care for his/her condition and patient is educated and empowered to be responsible for participating in the plan of care.Develops individualized patient/family education plan focused on self-management, delivers patient/family education specific to a disease state.Develops and updates training and educational materials and presents to appropriate staff, members and families.Facilitates patients return to normal daily activities by teaching and making appropriate referrals for outside services/continued care.Consults with internal and external physicians, health care providers, discharge planners, and outside agencies regarding continued care/treatment or hospitalization or referral to support services or placement.May need to facilitate transportation and housing arrangements for patient.Coordinates transmission of clinical and benefit treatment to patie

When we asked families what would improve their care, they asked for better continuity, care coordination, and communication. This is what I aim to provide by supporting and advocating for them during these long stays, and addressing psychological, social and medical concerns. Q: Tell us about a typical dayA: My role is incredibly varied and depends on the needs of each individual patient and their family. It can range from helping set up a bank account to helping manage an acutely deteriorating child needing intensive care. It often involves spending time with families, ward rounds, attending multidisciplinary meetings and teaching peers or parents.     Nurse demand is high, but job security isn't guaranteed. Whether new or experienced, here are five common reasons nurses get fired and how to avoid them.1. Lack of ProfessionalismPro-Tip:If you're losing your temper, politely excuse yourself. Ask co-nurses to cover your tasks while you regain composure. A short bathroom break often helps.2. Misuse of Social MediaPro-Tip:Avoid taking pictures of patients as it's unethical and can cost you your job and license. Also, refrain from posting inflammatory comments on social media. While you can express your opinions, be cautious about how and where you do so.3. Too Many AbsencesPro-Tip:If you’re feeling tired and burnt out about your job, it’s a better idea if you can ask for a few days off. Taking the time to relax and address your work-related stress can give you a chance to renew your energy and perspective about your work.4. Slipping DrugsPro-Tip:There are a myriad of ways to relieve pain without the use of strong pain medications. If non-pharmacological interventions don’t work, make it a point to consult a doctor before taking any medications. As a nurse, you should be aware of the negative implications of self-medication and drug misuse.5. Failure to Update LicensePro-Tip:License renewal is usually set at a fixed date. Setting up a reminder a few months or weeks before its actual expiry date can help you keep track of it. The urgent care provider will collaborate with the PCP and other members of the Integrated Care Team to ensure the visits support decreasing ER utilization, decreasing hospital admissions, and most importantly increasing member satisfaction.Duties and Responsibilities: Provide on-site clinical care to include:Acute onset of a new illness.Evaluation of acute exacerbations of chronic conditions.Shortness of breath without chest pain, change in mental status or new onset hypoxia.CHF exacerbationsAsthma/COPD exacerbationsBronchitis/pneumoniaHyperglycemia Elevated blood pressure Evaluation and treatment of a variety of infectionsNew or worsening aches or pains, wounds, depression and/or anxiety.Members we know will go to the ER if not seen today.Hospital follow-ups not able to be seen by PCP.Ensure coordination of care to include:Timely and accurate completion of chart documents.Work with Treatment and Triage Nurse to address sick calls and visits requiring Provider management.Receive input from the Core Clinical Team (PCP, Care Manager or SW) to define visit goals.Communicate important post visit follow up information to Core Clinical Team.Collaborate with Integrated Care Team (ICT) members (behavioral health, nutrition, health education) for problem solving and coordination of care.Identify and refer to specialty programs when instability or crisis occurs such as: abuse/neglect, interpersonal violence, significant and complex housing, and food insecurity.Implement transitional care protocols as part of warm hand offs between internal and external care providers.Help patients identify barriers to meeting health and life goals and connect patients with resources to navigate barriers.Adjust care plan following guidelines for evidenced based chronic disease management.Educate and coach for medication adherence and chronic disease self-management skill development.Monitor members at risk of being lost to care and implement interventions to prevent loss.Education & Experience Requirements:Certified Registered Nurse Practitioner or Physician Assistant in a field of Adult Medicine with Acute Care experience.ACLS certified.Ability to take a creative and innovative approach to probl
